Decent Health Care: Indecent Billing Practices

The Maine Department of Health and Human Services (DHHS) shut down MaineCare billing for the Augusta-based “Decent Health Care” operated by an Iraqi immigrant who previously faced, but was acquitted in, a federal prosecution

Decent Health Care, also known as Decent Care Services, received its suspension notice on June 30 after MaineCare’s Program Integrity Unit uncovered credible evidence that it was billing for services it never provided.

According to MaineCare payment records obtained by The Maine Wire, Decent billed $1,121,430 in 2024, its first year of operation, and then ramped up to billing $4,848,112 in 2025.

Decent is operated by Faysal Kalayaf Manahe (identified in some records as Faysal A. Kalayaf) and, according to business records, remains in good standing despite its funding shutoff.

The “business classification text” field on records that typically displays the type of business reads “I forgot to file the annual report” on Decent’s records.

Records show that Decent first incorporated in 2018 but nevertheless did not start billing MaineCare until years later in 2024.

Decent also maintains an active license to operate as a personal care agency and has held the license since 2024.

An inspection listed on the licensing records, conducted in March, just months before Decent’s billing was shut off, found a variety of deficiencies related to proper record-keeping and staff training.

Manahe submitted a plan of correction saying that he would remedy the issues. That plan was accepted by licensing inspectors.

The Maine Wire reached out to the number listed on licensing records for comment, but no one picked up, and no one called back after we left a voicemail.

Decent is located at 179 Mt. Vernon Ave. in Augusta.

The Maine Wire visited Decent’s office, which appeared to be closed down. It did, however, still have a sign up.

It shares its address with Sham Al Aseil Inc., a business founded in April of this year and operated by Moshirah Alrajab. It is not clear from its business registration what it is, and it appears to have no online presence outside of its registration.

Manahe does not appear as the registered agent for any other businesses, but he does appear on a federal indictment from 2021, when he, Yaser Aali, Ammar Alkinani, and Quasim Saesah were accused of a COVID-era scheme to eliminate competition with an agreement to fix the pay of home care workers and to avoid hiring each other’s employees.

The four men, identified by the American Bar Association as Iraqi immigrants, were ultimately acquitted.
