Top Republican budget officials have called for a thorough review by the Congressional Budget Office (CBO) of a recent proposal from the Biden administration that would provide financial relief to Medicare insurers.
While the Biden administration has described the one-time payments to insurers as a “demonstration program,” Republicans have alleged that bailout is an attempt to use taxpayer dollars to protect vulnerable Democrats on Election Day.
In a letter sent Monday, House Budget Committee Chairman Jodey Arrington (R-Texas) and Senate Budget Committee Ranking Member Chuck Grassley (R-Iowa) demanded and CBO review and highlighted Republicans’ criticism of the Medicare payouts.
“August 16, 2024, marked the second anniversary of the Inflation Reduction Act (IRA) being signed into law. Unfortunately, we continue to witness the adverse effects of Congressional Democrats pushing this partisan bill through, with Vice President Kamala Harris casting the deciding vote in the Senate,” Arrington and Grassley wrote.
“Because of Democrats’ rushed, partisan policymaking in the IRA, America’s seniors now face skyrocketing premiums for prescription drug coverage next year,” the Republicans said. “This will constrict their choices when making health coverage decisions for next year.”

“To cover up rising premiums ahead of November, the Biden-Harris Administration announced a new demonstration program, which will buy down premium increases by sending billions of taxpayer dollars directly to large insurance companies,” they said.
The GOP lawmakers argue that the IRA’s implementation has led to significant increases in premiums for prescription drug coverage under Medicare, limiting choices for seniors in the upcoming year. They contend that to mitigate the impact of these rising premiums, the Biden administration has introduced a demonstration program that channels billions of taxpayer dollars directly to major insurance companies.
The Biden administration’s initiative, announced through the Centers for Medicare and Medicaid Services (CMS), is designed to prevent proposed increases in Medicare Part D premiums from taking effect in the next year. Republicans, however, allege that the payments are actually intended to delay notifications of 2025 rate increases, which would typically be sent to Medicare recipients just before the November elections.
Earlier this summer, Medicare prescription drug insurers proposed rate hikes in response to provisions within the Inflation Reduction Act. The IRA, a key legislative achievement for President Biden, was passed in 2022 along strict party lines and included a $30 billion annual adjustment to Medicare Part D benefits.
The anticipated changes, set to take effect in 2025, have prompted insurers to increase premiums and reduce plan options for seniors. The Biden administration has responded by offering one-time payments to insurers totaling roughly $5 billion.
The CMS program, known as the Premium Stabilization Demonstration, would reduce the base beneficiary premium by $15 and cap the year-over-year increase of Part D premiums at $35. Critics argue that this shift in financial responsibility from insurers to taxpayers is both a political maneuver and a potential legal violation.
The controversy surrounding the IRA continues, with Democrats, including Maine Reps. Chellie Pingree and Jared Golden, remaining largely silent on the issue.
In their letter to CBO Director Phillip Swagel, Grassley and Arrington demanded an analysis of the budget impact of the so-called “Demonstration Program,” a breakdown of the projected payouts for insurers, and a comparison of how the IRA cost assumptions Congress voted on compare to the law’s actual costs.
Arrington and Grassley were joined in writing the letter by Senate Finance Committee Ranking Member Mike Crapo (R-Idaho), House Energy and Commerce Committee Chair Cathy McMorris Rodgers (R-Wash.), and House Ways and Means Committee Chairman Jason Smith (R-Mo.).
